Redemption Details
· After purchase is complete please provide consumer with redemption form and original sales receipt showing name of tires purchased and total purchase price. Taxes, fees and road hazard policies must be listed separately on the invoice.
· Invoice must show as paid and cannot be an estimate or work order.
· Consumer must then complete the redemption form and mail with original receipt to program headquarters.
· No online redemptions accepted for the gift with purchase offer
· Redemption must be postmarked by November 29, 2008
